2. We will accept assignment from insurance companies; however, the patient’s portion is due at the time of treatment. We accept assignment from insurance companies. We will gladly file claims to your dental carrier, and we accept payment from any carrier that offers assignment of benefits. However, you should note that regardless of what your insurance plan pays or intends to pay, you are responsible for your balance at the end of each appointment.
3. We require 48 hours notice for cancellations.
As a courtesy to our pre-scheduled patients, a phone call is requested to reduce waiting time for everyone. For emergencies outside of patient hours, please contact Dr. Jason Harvey at (780) 722 7175. Please note that during an Emergency Appointment, the dentist will relieve your discomfort and make a diagnosis regarding necessary treatment. At that time we would be happy to discuss treatment and fees.
As a courtesy, we are happy to assist our patients with dental benefits by filling out necessary forms, or submitting electronically where possible. However, due to the dramatic changes with insurance benefits, we ask that all treatment be paid for at the time of service and your dental insurance company will reimburse you directly. For your convenience, we accept cash, Visa, MasterCard, American Express, and Interac. We do not accept payment by cheque.
The billing of insurance companies directly can be arranged via pre-authorized credit card for any additional balance due. When extensive treatment is necessary, an estimate of the fee will be presented before the treatment begins. We ensure that you are comfortable with financial arrangements prior to treatment. Our team would be pleased to answer any questions you may have regarding fees, insurance, and financial arrangements. We are here to help you.
We thank you in advance for your cooperation.
